Tryout Forms

Attention Prospective Student-Athletes:

Please know that when you try-out for Coker College Athletics, the Athletic Training Staff needs certain information as mandated by the NCAA.  The following information is needed:

1. Physical, dated within 6 months of this try-out or within 6 months of athletic participation at his or her institution.  (NCAA Bylaw 13.11.2.1)
2. Official Medical documentation stating your sickle cell status
3. Waiver form for Coker College which can be downloaded Click Here.

This information MUST be presented to the Athletic Training Staff at least 24 hours prior to your try-out.  Failure to do so, may cancel your tryout.
