Letter from the Trainer
Dear Student-Athlete and Parent(s):
We are pleased to have you as a member of our athletics program at Coker College. We hope that you will find success and great enjoyment in your academic and athletic endeavors. As a participant in our Intercollegiate Athletic program, we carry a secondary insurance plan that covers you for athletic injuries subject to its limitations and conditions.
In order to be cleared for tryouts, practice and competition, each student-athlete must download the required medical and athletic forms located by following the link on this page. Please complete these forms and have them sent to the address below no later than July 15. Please include a photocopy of the front and back of your insurance card as well.
The purpose of these forms is to makes the Athletic Training Staff aware of any potential problems concerning the health of an athlete and helps to prevent delays in the event of an injury. All information revealed will be considered confidential and will not interfere with acceptance in the College unless such findings would endanger other students or staff. Legal safeguards make it necessary for each student-athlete to have a Medical Profile on file in the Athletic Trainers' Office.
We have two team physicians at Coker College. Dr. Robert Elder is a family medicine physician and works for The Medical Group here in Hartsville. Dr. David Wiley is an orthopedic surgeon and practices with Hartsville Orthopedics and Sports Medicine. If your insurance plan is a PPO and you want us to utilize a preferred provider, you will need to include a list of the approved providers for your plan. If Dr. Elder or Dr. Wiley is to be used as a provider, just note that on the insurance form.
We look forward to seeing each of you and welcome all of our student-athletes to the Coker College campus. If any problems or questions arise, do not hesitate to call the athletic training room (383-8390) and we will be happy to assist you.
Sincerely,
Coker College Sports Medicine
